
MASKS REQUIRED IN THE OFFICE
Effective Friday, April 24, 2020 @ 8:00am, the village of Oak Lawn requires all employees and visitors to wear a face covering in essential businesses. Acceptable coverings include, but not limited to, a non-medical grade mask, cloth or plastic mask, scarf, bandana, or handkerchief.
GCI Chicago asks for your cooperation in this matter. The office does not have Personal Protective Equipment (PPE) available for patient use. Please bring your own mask to your appointment.

As a reminder, we serve a population of patients who are at increased risk of contracting any virus and we ask that you monitor symptoms of any illness prior to coming to the office.
Please reschedule your upcoming appointment if you say “yes” to any of the following questions within the past 14 days:
1. Do you have a cough?
2. Do you have a sore throat?
3. Do you have shortness of breath or difficulty breathing?
4. Do you have a fever, chills, or repeated shaking with chills?
5. Do you have diarrhea?
6. Do you have new loss of taste and/or smell?
7. Do you have a have a headache or muscle pain?
8. Have you traveled outside your community?
9. Have you come into contact with anyone with a confirmed or pending case of COVID-19?

GUEST POLICY DURING CORONAVIRUS PANDEMIC
During this time, if you have an upcoming appointment, please do not bring a guest with you. Guests may assist patients with wheelchairs or walking to the suites, but then should wait in the car during the appointment. Our waiting rooms are closed.
CANCELLATION FEE DURING CORONAVIRUS PANDEMIC
As a courtesy to our patients, during this outbreak of COVID-19, we will waive our cancellation fee if you are experiencing influenza/virus symptoms. Please call the office at 708-422-3242 if you need to reschedule your appointment.
