Post-Op Recovery
Recovery from surgery can be a gradual process. We are available for assistance as you return to your normal activities. We have provided you with this set of discharge instructions that addresses some of the most common concerns after your surgery. Prepare for your First Appointment
Please arrive 20 minutes prior to your appointment to allow for additional check-in time. Bring in your insurance cards and photo identification at your visit. Download and complete the New Patient Form prior to your appointment to speed up the registration process.

Release of Information (ROI) in healthcare is essential to ensure the confidentiality of our patients while providing continuity of care. Please visit this website for further information on management of ROI.
Download the Gynecologic Cancer Institute of Chicago Release of Information Form and fax to our office at 708-422-3243 to release your information or to have information released to our clinic.
Patient Portal
Our patient portal is called My Care Plus and you must be registered with your email through the office to get started. If you do not have an account set up yet, please call us at 708-422-3242 to register for an account. You can access your portal at anytime by visiting www.mycareplusonline.com. You will have access to most test results and a messaging service to office staff.
