Insurance Information
We are contracted with most major insurance plans; however, it is the patient’s responsibility to check with their insurance company to determine if we are contracted providers and verify eligibility and benefits. Please call our office if you need assistance. Payment is due in full at the time of service.
Co-Pays and Past Due Balances
We will collect all co-payments and past due balances at the time of service. If you are unable to make payment at the time of service, we will reschedule your appointment
Medicare
We are currently accepting new Medicare patients. Because there are many Medicare insurance plans it is the patient’s responsibility to contact their Medicare insurance plan to verify that were are eligible providers.
Medicaid
We are contracted with a limited number of Medicaid plans. Please check with your Medicaid insurance plan to verify if we are eligible providers.
